Your attention is drawn to the fact that this indemnity form contains provisions that:
• limit MPs and/or other third parties’ liability towards you;
• place certain assumptions of risk on you (including certain undertakings and indemnifications) towards MP and/or other third parties; and
• includes certain admissions of facts.

I, the undersigned, hereby confirm that I wish to take part in the Event activities which Event activities may include, but are not limited to, team building activities, games and swimming.
DISCLAIMER CLAUSE I agree that MP, their agents, suppliers, officers, directors, employees, volunteers, and representatives, are not responsible for any death, illness, injury, loss or damage of any kind sustained by myself, any person or student on the way to or from, or at, the Event and all related activities, caused in any manner whatsoever.
DESCRIPTION OF RISKS I acknowledge that I am aware of the possible risks, dangers and hazards associated with my participation in the Event and all related activities of the trip, including the possible risk of severe or fatal injury to others and/or myself.
These risks include, but are not limited to:
1. TRAVEL: the risks associated with travel and all related activities including transport by public or private motor vehicle, buses, aircraft, etc.
2. WEATHER: any injury, illness, death, loss or damage resulting from exposure to weather elements; and/or
3. PHYSICAL: physical injuries such as muscular injuries, bruises, scrapes, cuts, sprains, dislocations, broken bones, and head, facial or dental injuries which might result from an accidental injury; illness resulting from food poisoning or parasites; allergic reactions; and the possibilities of physical confrontations whether caused by me or some other person or persons resulting in injuries and/or death.
I accept and assume all such risks, dangers and hazards and the possibility of personal injury, death, permanent disability, property damage or loss resulting thereof.
RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY In consideration of MP allowing my me to participate in the Event and all related activities, I agree as follows;
1.TO WAIVE ANY AND ALL CLAIMS that I have or may have against MP, and their agents, suppliers, officers, directors, employees, volunteers, and representatives as a result of my participation in the Event and all related activities;
2.TO RELEASE MP from any and all liability from any loss, damage, injury or expense that I may suffer as a result of my participation in the Event and all related activities due to any cause whatsoever;
3.TO INDEMNIFY AND HOLD HARMLESS MP from any and all liability for any damage including consequential damages to the property of MP or the Event premises, or personal injury to, any third party, resulting from my participation in the Event and all related activities;
4. IN ENTERING INTO THIS AGREEMENT, I am not relying upon any oral or written representations or statements made by MP other than what is set forth in this indemnity form.
ACKNOWLEDGEMENT, CONSENT AND DECLARTION
1. I acknowledge that participation in the Event and related activities is entirely at my own risk.
2. I have notified MP of all relevant allergies that I have as well as all relevant medical conditions including, but not limited to, epilepsy, heart conditions and asthma. In so far as any activities at the Event may cause harm to me, I will advise MP and not participate in that specific activity.
3. I have read and understood this indemnity form and that I have executed this indemnity form voluntarily, and agree that it is to be binding upon my heirs, executors, administrators, representatives and myself.
4. For the purpose of this indemnity form I understand that by typing my name below, this will constitute my electronic signature and confirmation and agreement with the provisions of this indemnity form and shall, for the purposes of validity, enforceability and admissibility, be conclusive evidence of my intention to be legally bound as if such signature had been written by hand.
